Introduction

Substance use can affect health, learning, relationships, safety, and future opportunities. In this aiMOOC, you will examine why people may use substances, how repeated use can change the brain and behavior, why adolescence is a particularly important period for prevention, and how people can get help and recover.

The course is designed for Grades 9–10. It does not diagnose anyone and it does not replace medical care. If this topic connects to a real concern in your life, talk with a trusted adult, school counselor, doctor, nurse, or another qualified health professional. If someone is unresponsive, has very slow or abnormal breathing, or may be in immediate danger, contact local emergency services at once.

Think before you begin: What messages about alcohol, nicotine, cannabis, prescription medicines, and other drugs do teenagers receive from friends, families, advertising, entertainment, and social media? Which messages are based on evidence, and which may be misleading?

The CDC video above places adolescent health in a wider context. As you watch, note how supportive families, schools, health education, and connected communities can strengthen healthy development.


Learning Goals

Understanding Substance Use and Addiction


Key Terms

Substance use means taking a psychoactive substance that can affect the brain or body. The term covers legal and illegal substances, including alcohol, nicotine, cannabis, prescription medicines used in ways that affect the brain, and other drugs.

Substance misuse often means using a substance in a way that creates avoidable risk. For prescription medicines, misuse can include taking someone else's prescription, taking more than prescribed, or taking a medicine for a different purpose than directed.

Substance use disorder, often shortened to SUD, is a medical condition in which a pattern of substance use causes significant problems or distress. It can range from mild to severe. Only a qualified health professional should diagnose it.

Addiction is commonly used for a severe pattern of compulsive substance seeking or use despite harmful consequences. Addiction is not simply a matter of weak character or poor willpower. It involves learning, motivation, brain function, behavior, and the person's environment.

Tolerance means that repeated exposure can reduce the effect of the same amount of a substance, so a person may feel that more is needed to get the previous effect.

Physical dependence means the body has adapted to repeated exposure to a substance. Stopping suddenly may produce withdrawal symptoms. Physical dependence can occur with some prescribed medicines even when they are taken correctly, so dependence is not automatically the same as addiction.

Withdrawal is a set of physical or psychological symptoms that can occur when a person who has become dependent reduces or stops a substance. Withdrawal differs by substance and can sometimes be medically dangerous, so professional advice may be important.

Recovery is an ongoing process of improving health and well-being and reducing or stopping harmful substance use. Recovery can include treatment, family support, peer support, school support, medication when appropriate, and changes in routines or environments.

Addiction Is More Than One Factor

No single cause explains why one person develops a substance use disorder and another person does not. Risk can be influenced by biology, age, stress, trauma, mental health, family relationships, peer groups, availability of substances, social norms, marketing, community conditions, and patterns of use. Protective factors can reduce risk and strengthen healthy choices.

The diagram illustrates three interacting areas: the individual, the environment, and the substance. A useful scientific question is not simply "Why did this person make a bad choice?" but "What combination of personal, social, and substance-related factors affected the situation?"


The Brain, Learning, and Motivation


Reward Pathways and Dopamine

Your brain has systems that help you notice important experiences, learn from them, and repeat behaviors that support survival and well-being. Dopamine is one chemical messenger involved in motivation, reinforcement, movement, attention, and learning. It is inaccurate to call dopamine only a "pleasure chemical"; its role is more complex.

Many addictive substances can strongly affect reward-related signaling. Repeated exposure can change how cues, expectations, and habits influence behavior. Places, people, moods, or objects associated with previous substance use may become powerful triggers. At the same time, repeated use can make ordinary rewards feel less motivating for some people.

Habit, Control, and Decision-Making

A behavior can become more automatic through repetition. In addiction, strong motivation to use a substance may compete with long-term goals such as health, school, sports, relationships, or work. Stress, sleep loss, emotional distress, and exposure to cues can make self-control harder.

This does not mean that a person's future is fixed. The brain remains capable of learning and change. Treatment and supportive environments can help people develop new routines, respond differently to triggers, and rebuild rewarding activities.


Why Adolescence Matters


A Brain Still Under Construction

Adolescence is a period of rapid physical, social, and brain development. Brain systems involved in planning, impulse control, weighing consequences, and long-term decision-making continue developing into the twenties. At the same time, social rewards, novelty, and emotionally important experiences can feel especially powerful.

The highlighted area is the prefrontal cortex, a region involved in planning, attention, working memory, and behavioral control. Brain development is not a simple "gas pedal versus brakes" story, but this region is one important part of the networks that support thoughtful decisions.

Because the adolescent brain is highly plastic, adolescence is also a strong period for positive learning. Skills such as emotional regulation, problem-solving, refusal strategies, healthy coping, sleep habits, sport, music, art, supportive friendships, and help-seeking can become powerful protective patterns.


Early Use and Later Risk

Research consistently shows that beginning substance use earlier in life is associated with a greater risk of later substance-related problems. This association does not mean that every young person who experiments will develop an addiction, and it does not prove that one factor alone causes later outcomes. Risk depends on many interacting factors.

For prevention, the practical message is clear: delaying or avoiding substance use during adolescence protects a period of important development and reduces exposure to avoidable harms.


Substances and Health Risks

Different substances affect the brain and body in different ways. Risk depends on the substance, dose, strength, frequency, age, health conditions, other substances used at the same time, and the situation. A legal product is not automatically safe, and a prescribed medicine is safe only when it is used as directed for the person to whom it was prescribed.

Alcohol

Alcohol is a psychoactive drug. It can impair judgment, reaction time, balance, memory, and impulse control. Drinking quickly or consuming a large amount can cause alcohol poisoning, which is a medical emergency. Because the adolescent brain is still developing, alcohol exposure during the teen years is a particular concern.

Alcohol rules differ among countries and regions. Regardless of local law, no one should drive, cycle in traffic, swim, operate machinery, or take other safety-critical actions while impaired.


Nicotine, Cigarettes, and Vaping

Nicotine is highly addictive. Most e-cigarettes contain nicotine, and youth can develop signs of nicotine dependence quickly. Nicotine exposure during adolescence can affect brain systems involved in attention, learning, mood, and impulse control.

Vaping aerosols are not simply harmless water vapor. They can contain nicotine, ultrafine particles, flavoring chemicals, and other substances that may affect the lungs and cardiovascular system.

The CDC video models a supportive conversation about vaping. Notice that the adult does not rely only on fear. What communication choices make the conversation more likely to continue?

Cigarette smoking also damages many organs and increases the risk of cancer, heart disease, stroke, and lung disease.


Cannabis

Cannabis can affect memory, attention, reaction time, coordination, decision-making, and perception. These effects can make driving and other safety-sensitive activities dangerous. In adolescents, frequent cannabis use is associated with a higher risk of cannabis use disorder and with problems in learning and school performance.

Cannabis can also cause anxiety, paranoia, or temporary psychotic symptoms in some people. Research links earlier and more frequent use with a stronger association with psychotic disorders, although individual risk varies and not every person who uses cannabis will develop these problems.

Laws and medical regulations for cannabis vary by location. Legal status does not determine whether a substance is safe for a developing brain.


Prescription Medicines and Counterfeit Pills

Prescription medicines can be valuable treatments when they are prescribed to the right person and taken exactly as directed. Misuse includes taking someone else's medicine, changing the dose without medical guidance, or using a medicine to change mood or get high.

A pill obtained outside a legitimate pharmacy cannot be assumed to contain what its appearance suggests. Counterfeit pills may contain unexpected substances, including illegally manufactured fentanyl, which can cause fatal overdose in very small amounts. The safest rule for a medicine is simple: take only medicines prescribed or recommended for you by a qualified professional and supplied through a legitimate source.

This short CDC video warns about counterfeit pills and hidden fentanyl. Its message is deliberately direct because an unexpected opioid can stop breathing.


Mixing Substances Increases Risk

Combining substances can produce effects that are stronger and less predictable than using one substance alone. Mixing alcohol, opioids, or sedatives can dangerously slow breathing. Combining stimulants with other substances can increase strain on the heart and make impairment harder to judge.

Never treat "balancing" one drug with another as a safety strategy. If a prescribed medicine may interact with another medicine or substance, ask a pharmacist or healthcare professional.


Risk and Protective Factors


Risk Is Not Destiny

A risk factor is something associated with a greater chance of a harmful outcome. A protective factor is something associated with lower risk or greater resilience. These factors operate at several levels.

Risk factors should never be used to label a person as "doomed" to develop addiction. Protective factors can be strengthened, and people can recover even after serious problems have developed.


School Connectedness as Prevention

Feeling that adults and peers at school care about you can protect health in several ways. A connected student is more likely to have someone to ask for help, more opportunities for meaningful participation, and fewer reasons to cope alone.

A useful prevention question is: "What can our school change so healthy choices become easier?" This shifts attention from blaming individuals toward building supportive environments.


Decision-Making, Peer Pressure, and Media Literacy


A Simple Decision Check

When you face a risky situation, slow the decision down. Ask: What is being offered? What do I actually know about it? What could happen in the next hour? What could happen tomorrow? Is anyone pressuring me? What would make it easier to leave? Which trusted person could I contact?

You do not need a dramatic speech to refuse. Clear phrases such as "No thanks," "I'm not doing that," "I have practice tomorrow," or "I'm leaving" can work. Repeating the same answer and moving toward supportive people can be more effective than arguing.

A practical safety plan can include a charged phone, a trusted contact, a way to leave, and an agreement that asking for help will be taken seriously.


Advertising and Social Media

Marketing often connects products with independence, popularity, attractiveness, stress relief, rebellion, or fun. Influencers and entertainment may also normalize substance use without showing long-term consequences.

Use these media-literacy questions:

  1. Source evaluation: Who created this message, and what do they gain if I believe it?
  2. Evidence: What evidence is shown, and what important information is missing?
  3. Framing: Which emotions, images, music, or social signals are used to influence me?
  4. Comparison: Does a reliable health source support the claim?
  5. Uncertainty: Is the message more confident than the evidence allows?

A strong health claim should be checked against current information from recognized public-health or medical organizations, not just testimonials, viral posts, or product advertising.


Recognizing When Someone May Need Help

A single sign does not prove that someone has a substance use disorder. Changes can have many causes. Concern grows when several changes appear together, continue over time, or create danger.

Possible warning signs include losing control over use, repeated unsuccessful attempts to cut down, strong cravings, using despite school or relationship problems, giving up valued activities, taking serious risks while impaired, needing more for the same effect, or experiencing withdrawal.

Other signs may include sudden changes in attendance, grades, sleep, mood, money, friendships, secrecy, or health. These signs should lead to a supportive conversation, not public accusation.

A helpful response is to describe what you have noticed, listen without humiliation, encourage a trusted adult or health professional, and take immediate safety threats seriously. Do not promise to keep a dangerous situation secret.


Overdose and Emergency Response

An overdose happens when a substance or combination of substances overwhelms the body's ability to function safely. Any suspected overdose should be treated as a medical emergency.

For a possible opioid overdose, warning signs can include inability to wake, very slow or shallow breathing, choking or gurgling sounds, and discolored lips or nails. If you are unsure, it is safer to seek emergency help.

Emergency priorities: call your local emergency number, follow the dispatcher's instructions, stay with the person if it is safe to do so, and use an opioid-overdose reversal medicine such as naloxone if it is available and you know how to follow the product or local emergency guidance. Naloxone can temporarily reverse an opioid overdose, but emergency medical care is still needed.

Never assume that someone can "sleep off" severe unresponsiveness or abnormal breathing. Do not make the person walk, put them in a cold shower, or give them food or drink. The goal is rapid professional help and support of breathing.


Treatment, Recovery, and Support

Substance use disorders are treatable. Effective care depends on the person, substance, severity, age, mental health, family situation, and other needs. Treatment can include behavioral therapies, family-based approaches, school support, peer recovery support, and medications for some substance use disorders.

For adolescents, involving supportive family members or caregivers is often important. Treatment may also address anxiety, depression, trauma, learning difficulties, conflict, housing, or other problems that affect recovery.

The NIDA video explains why stopping can be difficult and why support matters. A return to use does not erase earlier progress. Healthcare teams may adjust treatment, identify triggers, and strengthen the recovery plan.

Recovery is not one identical path for everyone. Success may include improved health, safer behavior, restored relationships, regular school attendance, new coping skills, and a growing sense of purpose.


Language, Stigma, and Respect

Words can affect whether people seek help. Person-first language emphasizes that a person is more than a diagnosis or behavior.

Prefer phrases such as person with a substance use disorder, person who uses substances, or person in recovery. Avoid labels such as "junkie," "druggie," or other insults. Avoid treating addiction as evidence that someone is morally bad.

Respectful language does not mean ignoring risky behavior. You can clearly name harm, boundaries, and responsibility while still recognizing a person's dignity and capacity to change.


Summary of Core Ideas

Substance use and addiction involve interactions among the brain, behavior, development, social environments, and the properties of substances. Adolescence is a period of opportunity and vulnerability because the brain and social world are changing quickly. Early prevention, supportive relationships, accurate information, and school connectedness can reduce risk.

Addiction is not identical to tolerance or physical dependence. A substance use disorder is a medical condition that can be treated. Media messages should be evaluated critically, and claims about "safe" products should be checked against reliable evidence.
