Introduction
Clinical psychology is a scientific and professional field concerned with understanding, assessing, preventing, and treating mental, emotional, behavioral, and interpersonal difficulties. It brings together psychological science, psychological assessment, psychopathology, psychotherapy, prevention, consultation, supervision, and research. Clinical psychologists may work with individuals, couples, families, groups, organizations, and communities across the lifespan.

Foundations and Scope
What Clinical Psychologists Do
Clinical psychology covers a wide range of activities. Psychologists may conduct clinical interviews, administer and interpret psychological tests, observe behavior, assess risk, develop case formulations, deliver psychological interventions, evaluate outcomes, consult with other professionals, supervise trainees, teach, design services, and conduct research.
The exact legal scope of practice varies by country and jurisdiction. In many settings, clinical psychologists collaborate with psychiatrists, physicians, nurses, occupational therapists, social workers, speech and language professionals, educators, and other specialists. Psychiatrists are medical doctors; psychologists are trained primarily in psychological science and psychological methods. Prescribing rights for psychologists are not universal and depend on local law and additional training.
Clinical Psychology and Related Fields
Clinical psychology overlaps with counseling psychology, health psychology, neuropsychology, school psychology, psychiatry, and social work, but these professions are not identical. The boundaries differ internationally. This means that evidence-based care is not a mechanical choice from a treatment list. It requires judgment about whether evidence applies to a particular person and setting, as well as ongoing monitoring of progress.
Important forms of evidence include controlled trials, systematic reviews, meta-analyses, observational studies, qualitative research, single-case designs, measurement studies, process research, and implementation research. Different questions require different methods.

Models of Mental Health and Disorder
Biopsychosocial Thinking
Clinical problems are usually multi-determined. A biopsychosocial model considers interacting biological, psychological, and social influences. Biological factors can include genetics, sleep, pain, medication effects, illness, and neurodevelopment. Psychological factors can include learning history, attention, beliefs, emotion regulation, coping, memory, and behavior. Social factors can include relationships, discrimination, economic conditions, housing, education, work, community, and culture.

The model is useful because it discourages single-cause explanations. It does not imply that every factor is equally important in every case. A good formulation identifies the factors most relevant to the specific problem and tests whether changing them improves functioning.
Dimensional and Categorical Perspectives
A categorical approach asks whether a person meets criteria for a defined disorder. A dimensional approach asks where a person falls on continua such as anxiety, mood disturbance, impulsivity, or functioning. Clinical psychology often uses both. Categories can support communication, service planning, and research, while dimensions can capture severity and variation that categorical labels may miss.
You should avoid treating a diagnosis as a complete explanation of a person. Diagnosis can summarize a pattern of difficulties, but it does not by itself explain why the pattern developed, what maintains it, what strengths the person has, or which intervention will work best.
Psychological Assessment
Psychological assessment is a structured process of gathering and integrating information to answer a referral question. Strong assessment is multi-method and, when appropriate, multi-informant. Sources may include interviews, questionnaires, behavioral observations, cognitive tests, personality measures, records, collateral information, and medical data.
Reliability, Validity, and Norms
A measure is useful only if its scores can be interpreted responsibly. Reliability concerns consistency or precision. Validity concerns whether evidence supports the intended interpretation and use of scores. Norms allow a score to be compared with an appropriate reference group. Good assessment also considers measurement error, base rates, cut scores, language, disability access, cultural relevance, and the consequences of decisions.
A statistically unusual score is not automatically evidence of a disorder. Interpretation depends on the referral question, the quality of the measure, the comparison group, the person's context, and converging evidence from other sources.
Interviews and Behavioral Assessment
Clinical interviews can be unstructured, semi-structured, or structured. Structured and semi-structured approaches can improve consistency, but clinical judgment remains necessary to clarify meaning and context. Behavioral assessment focuses on observable patterns and the situations in which they occur. Functional analysis may examine antecedents, behavior, and consequences to identify maintaining processes.
When an assessment question could have medical or neurological explanations, clinical psychologists may recommend appropriate medical evaluation rather than assuming that all symptoms are psychological.
Historical and Contemporary Testing
Projective methods such as the Rorschach test are historically influential. Their contemporary use is debated and requires specialist training, standardized administration, appropriate scoring systems, and careful attention to the evidence for the specific interpretive claim. No responsible assessment should diagnose a person from an isolated response to an image.

Diagnosis and Classification
Diagnostic classification provides shared language for describing clinically significant patterns of symptoms and impairment. Two major systems are the World Health Organization's ICD-11 and the American Psychiatric Association's DSM-5-TR. Their categories overlap substantially but are not identical in structure or wording.
The ICD-11 is a global health classification system used for recording and reporting diseases and health conditions. Its clinical descriptions and diagnostic requirements support mental, behavioral, and neurodevelopmental diagnosis across countries. The DSM-5-TR is widely used in the United States and in many research and training settings.

Differential Diagnosis and Comorbidity
Differential diagnosis means comparing plausible explanations for a person's presentation. Clinicians consider whether symptoms are better explained by another psychological condition, a medical condition, substance effects, developmental factors, environmental stress, culturally normative experiences, or combinations of these.
Comorbidity refers to the presence of more than one diagnosable condition. High comorbidity is one reason that formulation and dimensional assessment are important: two people with the same diagnosis may have very different mechanisms, strengths, and treatment needs.
Case Formulation
A case formulation is a working explanation of how a person's difficulties developed and what currently maintains them. Unlike a diagnosis, a formulation is individualized and explicitly connected to intervention planning.
One common framework organizes information into predisposing factors, precipitating factors, perpetuating factors, protective factors, and the presenting problem. This is sometimes called a five-part or "5 Ps" formulation. Other models derive from cognitive-behavioral, psychodynamic, systemic, developmental, trauma-informed, or integrative theories.
A strong formulation is not a story that merely sounds plausible. It should generate testable predictions. If a proposed maintaining factor is changed, the clinician should expect a measurable change in the target problem. Outcome data can therefore support, refine, or challenge the formulation.
Psychological Interventions
Clinical psychologists use a range of psychological interventions. Measurement-based care does not reduce people to scores. Instead, scores become one source of information in an ongoing conversation. Culture and context shape how distress is experienced, described, interpreted, and treated. Clinicians should consider language, migration, disability, gender, socioeconomic conditions, family systems, religion or spirituality when relevant to the person, discrimination, historical context, and access to resources.

An ecological perspective emphasizes that individuals are embedded in relationships, institutions, communities, and broader social systems. This can prevent an overly individualistic explanation of problems that are partly produced or maintained by environmental conditions.
Cultural humility involves sustained self-reflection and openness rather than assuming mastery of another person's culture. Clinicians should ask what experiences and identities are meaningful to the person, avoid stereotyping, and evaluate whether assessment tools and interventions are valid and accessible for the population in question.
Clinical Research and Evidence
From Research Question to Inference
Clinical research begins with a clear question. Questions about treatment efficacy may be addressed with randomized controlled trials. Questions about prognosis may use longitudinal designs. Questions about lived experience may use qualitative methods. Questions about mechanisms may require experimental or mediation designs. Questions about real-world implementation may use pragmatic trials or implementation science.
Important concepts include sampling bias, confounding, effect size, statistical uncertainty, missing data, preregistration, replication, and external validity. A statistically significant result is not automatically clinically important. You should examine effect size, precision, comparison conditions, harms, adherence, follow-up, and relevance to the target population.
Systematic Reviews and Meta-Analysis
A systematic review uses explicit methods to locate, evaluate, and synthesize studies. A meta-analysis statistically combines compatible effect estimates. These approaches can strengthen inference, but their conclusions still depend on the quality, comparability, and publication status of the underlying studies.
University students should learn to distinguish a strong systematic review from a narrative claim that merely cites several studies. Look for transparent search methods, inclusion criteria, risk-of-bias assessment, heterogeneity analysis, and discussion of limitations.
Clinical Neuroscience
Clinical psychology increasingly interacts with neuroscience, genetics, psychophysiology, and computational methods. These fields can help explain mechanisms and individual differences, but biological data should not be interpreted as automatically more objective or more clinically meaningful than behavioral and contextual data.

Functional magnetic resonance imaging can identify patterns of brain activity related to tasks or states. However, fMRI is not a routine stand-alone diagnostic test for most common mental disorders. Group-level research findings often do not translate directly into accurate classification or treatment prediction for an individual person.
