Emergency Nursing



Introduction

Emergency Nursing is the nursing specialty concerned with rapid assessment, prioritization, stabilization, ongoing monitoring, communication, and compassionate care for people with acute illness or injury. Emergency nurses work with undifferentiated patients: the diagnosis may be uncertain, information may be incomplete, and a patient's condition can change within minutes. Your task is therefore not only to perform procedures, but also to recognize risk, organize care, communicate clearly, and repeatedly evaluate whether the patient is improving or deteriorating.

Emergency care is delivered in emergency departments, trauma centers, urgent and acute-care units, ambulances, disaster settings, and other locations where time-sensitive illness or injury is managed. The World Health Organization's Basic Emergency Care approach emphasizes systematic assessment, early recognition of life-threatening problems, immediate action within the clinician's scope, and reassessment.

The Emergency Nursing Role

Emergency nursing combines speed with disciplined assessment. You may need to receive a prehospital handover, identify an immediate threat, begin monitoring, prepare equipment, obtain vascular access, administer prescribed or protocol-based treatment, support relatives, document events, and coordinate with several professions at the same time. Safe performance depends on prioritization rather than simply doing tasks quickly.

Common emergency nursing responsibilities include rapid assessment, triage, repeated vital-sign measurement, pain assessment, medication safety, specimen collection, wound and device care, resuscitation support, infection prevention, safeguarding, discharge teaching, and transfer or admission handover. Advanced roles vary by country and institution.


A Structured First Look

Before touching the patient, consider scene and staff safety, infection risks, the need for personal protective equipment, and whether extra help is required. Observe the patient's general appearance, work of breathing, color, ability to speak, posture, behavior, and responsiveness. A patient who looks critically ill should not wait for a complete history before immediate assessment and treatment begin.

Your first assessment should answer three questions: Is there an immediate threat to life? What must be done now? What information and resources are needed next?


Triage and Prioritization

Triage sorts patients according to clinical urgency and resource need rather than order of arrival. Many emergency systems use five acuity levels, but the names and criteria differ between countries and organizations. For example, the Emergency Severity Index and the Canadian Triage and Acuity Scale are both five-level systems. You must use the validated system adopted by your service.

A triage decision is provisional. A patient waiting for care can deteriorate, so reassessment is essential. New chest pain, falling blood pressure, increasing respiratory effort, altered consciousness, new neurological deficit, severe bleeding, or rapidly worsening pain should trigger prompt escalation according to local policy.

Routine emergency-department triage is different from mass-casualty triage. In a mass-casualty incident, limited resources and incident-management goals change how priorities are assigned. Never transfer a routine ED triage rule directly into disaster practice without the appropriate disaster protocol.


Triage Reasoning

Useful triage questions include: What is the chief concern? When did it begin? What has changed? Are there red flags? What are the vital signs? Is the patient vulnerable because of age, pregnancy, disability, communication barriers, intoxication, immunosuppression, or social circumstances? What treatment has already been given?

A strong triage nurse combines objective findings with pattern recognition but avoids relying on intuition alone. Vital signs, mental status, pain, mechanism of injury, comorbidity, and observed deterioration all contribute to risk.


The ABCDE Primary Assessment

The ABCDE approach provides a systematic method for assessing and treating a critically ill or injured patient. Work through Airway, Breathing, Circulation, Disability, and Exposure. Treat life-threatening problems as you find them, ask for help early, use the team in parallel, and reassess after every important intervention.


Airway

Assess whether the airway is open and protected. Listen for abnormal sounds, observe whether the patient can speak, and consider risks such as reduced consciousness, facial injury, swelling, secretions, vomit, or foreign material. Use appropriate basic airway maneuvers and suction within your competence, prepare airway equipment, and escalate immediately if patency or protection is threatened.


Breathing

Assess respiratory rate, effort, chest movement, oxygen saturation, skin color, breath sounds when appropriate, and the patient's ability to speak. Severe respiratory distress can exist even before oxygen saturation falls. Give oxygen or ventilatory support according to the clinical situation and local protocol, and evaluate the response.

Pulse oximetry is useful but not sufficient by itself. Read the number in context: poor perfusion, motion, nail products, device limitations, and dyshemoglobinemias can affect interpretation. Trends and the patient's clinical condition matter.


Circulation

Assess pulse, blood pressure, capillary refill where appropriate, skin temperature, bleeding, mental status, and other signs of perfusion. Identify external hemorrhage and control it promptly. Prepare monitoring, vascular access, blood sampling, fluid or blood-product support, and other treatments according to the cause of shock and local protocol.

Shock is a state of inadequate tissue perfusion and may be hypovolemic, distributive, cardiogenic, or obstructive. A normal single blood-pressure reading does not exclude early shock. Look for trends and for signs such as tachycardia, altered mental status, cool or mottled skin, weak pulses, reduced urine output, or rising respiratory effort.


Disability

Assess neurological function rapidly. Level of consciousness, pupil findings, movement, speech, seizure activity, and bedside glucose can identify immediately reversible or time-critical problems. A sudden change in mental status is an emergency finding, not simply a behavioral problem.

Useful structured tools may include the Glasgow Coma Scale and simple responsiveness scales, but the most important observation is change over time.


Exposure and Environment

Expose enough of the patient to identify hidden injury, rash, bleeding, burns, medical devices, or other clues while protecting dignity and preventing hypothermia. Environmental control matters: trauma patients, older adults, infants, and people with shock can lose heat rapidly.


Reassessment

ABCDE is a cycle, not a one-time checklist. Return to the beginning after an intervention, after transfer, when new information appears, or whenever the patient changes. Reassessment allows you to judge whether treatment is working and whether a new threat has developed.


Monitoring and Diagnostic Support

Emergency nursing surveillance combines measurements with bedside observation. Common tools include non-invasive blood pressure monitoring, pulse oximetry, cardiac monitoring, electrocardiography, temperature measurement, glucose testing, capnography in selected patients, and laboratory or imaging support.

A monitor can alert you to risk, but it cannot replace clinical assessment. Check whether the displayed rhythm fits the patient's pulse and condition, investigate implausible readings, and respond to the patient rather than the alarm alone. Artifact, disconnected leads, poor sensor contact, and movement are common causes of misleading data.


Documentation as a Clinical Tool

Emergency documentation should make the timeline of care understandable. Record important assessments, changes in condition, interventions, medication administration, communication, escalation, patient response, and disposition according to local standards. In time-critical care, accurate timestamps can be especially important.

Documentation also supports continuity, audit, quality improvement, safeguarding, and legal accountability. Avoid copying forward information that you have not verified.


High-Risk Presentations

Emergency nurses encounter a broad range of undifferentiated problems. The aim of early nursing care is not to make every diagnosis independently, but to identify danger, initiate appropriate immediate actions, support diagnostics and treatment, and detect change.

Severe Bleeding and Trauma

In trauma, uncontrolled hemorrhage can become fatal quickly. Apply appropriate direct pressure, wound packing, or tourniquet techniques according to injury type, training, and protocol. In a hospital setting, these measures are integrated with trauma-team activation, resuscitation, diagnostics, warming, blood-product systems, and definitive hemorrhage control.


Sepsis and Shock

Sepsis is life-threatening organ dysfunction caused by a dysregulated response to infection. The emergency nurse contributes to early recognition, escalation, specimen collection, antimicrobial and fluid pathways as prescribed or protocolized, perfusion monitoring, and repeated evaluation of response.

Do not reduce sepsis assessment to one score or one vital sign. Clinical context, trends, organ dysfunction, perfusion, respiratory status, mental state, and local sepsis pathways all matter. In shock, treatment must match the likely mechanism and the patient's response.


Chest Pain and Cardiac Emergencies

Chest pain can arise from cardiac, vascular, respiratory, gastrointestinal, musculoskeletal, and other causes. Some dangerous causes may present atypically. Early nursing priorities include ABCDE assessment, timely electrocardiography where indicated, continuous monitoring for unstable patients, vascular access and sampling as required, symptom assessment, and prompt escalation.

Cardiac arrest requires immediate recognition, high-quality cardiopulmonary resuscitation, rapid defibrillation when indicated, and coordinated team action according to current resuscitation guidelines.

Neurological Emergencies

Acute stroke, seizure, intracranial hemorrhage, meningitis, toxic-metabolic disturbance, hypoglycemia, and other conditions can present with altered consciousness or focal neurological signs. Establish the time course, check glucose promptly, protect airway and breathing, observe for seizure activity, document neurological changes, and activate appropriate time-critical pathways.

Never assume confusion is caused by age, intoxication, or psychiatric illness until dangerous medical causes have been considered.


Respiratory Emergencies

Asthma, chronic obstructive pulmonary disease exacerbation, pneumonia, pulmonary edema, pulmonary embolism, anaphylaxis, pneumothorax, and airway obstruction can all produce respiratory distress. Your assessment should integrate respiratory rate, work of breathing, oxygen saturation, auscultation when appropriate, circulation, mental state, and response to therapy.

A falling respiratory rate in an exhausted patient can signal deterioration rather than improvement. Escalate when effort, gas exchange, consciousness, or perfusion worsens.


Medication and Procedure Safety

Emergency departments are high-risk medication environments because decisions are time-sensitive, interruptions are common, and patient information may be incomplete. Verify identity, allergies, indication, drug, dose, route, timing, and relevant monitoring according to your local medication-safety standards. Clarify unclear prescriptions and use independent double checks where required.

High-alert medications, weight-based pediatric dosing, infusions, anticoagulants, sedatives, insulin, and vasoactive drugs require particular care. Use approved references and smart systems where available rather than relying on memory alone.

Procedures such as vascular access, urinary catheterization, wound care, splinting, specimen collection, airway support, and monitoring should be performed with aseptic technique, infection-prevention measures, consent where possible, and clear documentation.


Communication and Teamwork

Emergency care is interprofessional. Errors can occur when information is delayed, incomplete, ambiguous, or not acknowledged. Use concise structured communication, state urgency explicitly, and confirm critical information.

SBAR organizes a message into Situation, Background, Assessment, and Recommendation. Closed-loop communication adds confirmation: one team member gives a clear message, the receiver repeats or acknowledges it, and the sender confirms that the message was understood correctly.


Handover

A safe handover should communicate the patient's identity, reason for care, relevant history, key findings, current acuity, treatment already given, response, pending tasks or results, risks, and what the receiving team needs to do next. Handover is not only a transfer of information; it is a transfer of responsibility.

At the bedside, involve the patient when appropriate while protecting confidentiality and avoiding distressing discussion that should occur privately.


Escalation and Speaking Up

If you are concerned that a patient is deteriorating, communicate early and clearly. State the concern, provide supporting observations, recommend or request the next action, and repeat the escalation through the appropriate chain if the response is inadequate. Patient safety takes priority over hierarchy.

Team briefings before high-risk events and debriefings afterward can improve shared understanding, learning, and psychological safety.


Person-Centered and Ethical Emergency Care

Urgency does not remove the need for dignity, consent, privacy, cultural respect, and communication. Explain what you are doing whenever possible. Use qualified interpreters when language barriers affect informed decision-making. Adjust communication for hearing, vision, cognitive, developmental, or literacy needs.

Emergency nurses may encounter safeguarding concerns, sexual assault, domestic violence, neglect, human trafficking, intoxication, homelessness, and acute mental-health crises. Follow local legal duties and specialist pathways while avoiding judgmental language.

Pain and anxiety deserve assessment and treatment even when the patient is stable. At the same time, medication decisions must consider respiratory status, hemodynamics, allergies, interactions, intoxication, and other risks.


Family and Carer Involvement

Relatives and carers can provide essential history, baseline information, medication lists, and support. They may also be frightened or distressed. Give clear updates within privacy rules, identify a spokesperson when useful, and explain delays or changes in plan without making promises that cannot be kept.


Special Populations

Children, pregnant patients, older adults, people with disabilities, and patients with complex chronic disease may deteriorate differently or require adapted assessment. Pediatric vital-sign ranges are age-dependent, and many medication doses are weight-based. Pregnancy changes physiology and can affect positioning, differential diagnosis, imaging decisions, and resuscitation priorities. Older adults may have atypical presentations and higher risk from polypharmacy, falls, delirium, and functional decline.

Equity is a clinical safety issue. Bias can affect pain treatment, triage, communication, and diagnostic pathways. Use standardized assessment, objective data, reflective practice, and patient-centered communication to reduce avoidable disparities.


Infection Prevention and Occupational Safety

Emergency nurses often assess patients before an infectious diagnosis is known. Apply standard precautions consistently and add transmission-based precautions when indicated. Hand hygiene, appropriate personal protective equipment, sharps safety, respiratory hygiene, environmental cleaning, and prompt exposure management protect patients and staff.

Quality improvement may examine door-to-assessment time, pain reassessment, sepsis pathway compliance, medication errors, return visits, falls, patient experience, handover quality, and other locally relevant measures. Data should be used to understand processes and improve care rather than simply to blame individuals.

A useful reflective question is: What conditions made the safe action easy or difficult? This shifts attention from individual memory toward team design, workflow, communication, and resilience.
